There are two key failure modes when doing a Kanban adoption according to David J. Anderson: Overreaching and False Summit Plateau.

Overreaching: The implementation does not fit to the status quo of the current organization. It is too much and too difficult to be adapted.

False Summit Plateau: There is a white board with sticky notes and the people say: "We've done Kanban, what's next?"
